The first step is to make an appointment with our HUD-certified counselor, who will evaluate your current situation and make a determination on what course of action needs to be taken. You will need to submit copies of all the documents from the list below for our counselor to have all the information needed to make the best decision.

Loan Note: It’s the 3-6 page document you signed at closing that details the terms of your loan. If not available, you can request it from your mortgage lender and have it mailed to you.
Homeowners insurance Declarations page: If not available, you may request it from your insurance company or mortgage lender and have the document mailed to you.
Mortgage Statement(s): provide ONLY the latest statement (and other lien most recent statement)
Valid Photo ID: a copy

Paystubs: a copy of your most recent paystubs for the last month or other income documents such as a Social Security Benefits Letter, Pension Letter, Food Stamps, etc.
Rental/Lease Agreement: and proof of two months’ deposits (if applicable)
Contribution Letter: from any family members who are living on the property (if applicable)
Year-to-Date Profit & Loss: a copy of the statement (if self-employed)
Bank Statements: a copy for the past three months
Personal Income Tax Returns: a copy of the latest two years and business income tax, if applicable. ONLY federal returns with all schedules

Utility Bills: A copy of your most water, gas, electricity, and cable/phone bills
Transportation: a copy of your car loan/lease and insurance bills (if applicable)

Write a detailed explanation letter describing the hardship that has led you to request assistance. Write it in attention to your mortgage/lien company, and be sure to include specific dates (if available or at least month/year), if you want to keep your home or not, and what steps you have taken to improve your situation. If you have not had a hardship, you must explain why you request assistance.
